Posted Date: 11/13/2025
Vandalia Health Mon Medical Center earned an ‘A’ Hospital Safety Grade from The Leapfrog Group, a national nonprofit watchdog that sets standards for excellence in patient safety. The ‘A’ grade recognizes Mon Medical Center’s continued achievement in protecting patients from preventable harm and delivering the highest quality of care. This marks the hospital’s second consecutive ‘A’ grade from The Leapfrog Group and one of multiple ‘A’ grades earned in recent years.
Leapfrog assigns letter grades to general hospitals across the country based on more than 30 measures of errors, accidents, injuries and infections, as well as the systems hospitals have in place to prevent them.
“Receiving an ‘A’ for the second consecutive grading period demonstrates the continued commitment of our entire team to prioritize patient safety,” said David Goldberg, President and CEO of Mon Health System and Davis Health System–Vandalia Health Northern Region, and Executive Vice President of Vandalia Health. “Every department and every colleague, clinical and nonclinical alike, plays an essential role in achieving this result. It’s a reflection of our shared mission to deliver safe, quality care to our patients and community.”
“Safety isn’t achieved once — it’s sustained through daily vigilance, teamwork and a culture that puts our patients first,” said Krystal Atkinson, DNP, RN, CNOR, Chief Administrative Officer, Mon Medical Center, and Chief Nursing Executive, Vandalia Health Northern Region. “This recognition reflects the pride our teams take in caring for our community and their shared responsibility to deliver excellent outcomes with compassion and skill.
“Behind this grade is a deliberate and ongoing focus on strengthening our safety systems. Over the past year, we’ve taken meaningful steps to enhance that commitment — upgrading our Emergency Department with non-skid flooring to help prevent falls, expanding our ‘Speak Up’ patient voice campaign alongside a full-time Rapid Response Team to identify and respond to risks sooner, and reinforcing our culture of safety through daily shift huddles, good-catch recognition and continuous reporting. These initiatives together ensure that safety is not just a goal, but an integrated part of our daily operations.”
“Everyone who works at Vandalia Health Mon Medical Center should be proud of this ‘A’ Hospital Safety Grade,” said Leah Binder, president and CEO of The Leapfrog Group. “It takes complete dedication at every level, and an ironclad commitment to putting patients first. I thank Vandalia Health Mon Medical Center, its leadership, clinicians, staff and volunteers for caring so deeply for its patients and their safety.”
