Employed Physicians and Advanced Practice Providers may request privileges at one or more Vandalia Health hospital(s) via the initial appointment process. Initial appointment requires submission of a complete credentialing application, addendum, and the relevant Delineation of Privileges form(s) for the requested hospital(s).
