Activation at Additional Vandalia Health Hospital

Physicians and Advanced Practice Providers currently appointed to at least one Vandalia Health Hospital may request privileges at another Vandalia Health hospital via activation. Activation requires submission of the relevant Delineation of Privileges form for the requested hospital(s).
