Additional Privileges for Existing Appointment

Physicians and Advanced Practice Providers currently appointed at Vandalia Health Hospitals may request additional privileges at any Vandalia Health hospital to which they are currently appointed. Additional privileges require submission of the relevant Delineation of Privileges form and any additional supporting documentation for the requested hospital(s).
